  • severe heart failure
  • filariasis - damage to the lymph nodes located in the groin area with parasites, as a result of which there is a violation of the outflow of lymph from the testicles and dropsy develops,
  • tumorous growths in the scrotum,
  • complications after surgery in the scrotum (e.g. after treatment of a groin hernia),
  • severe liver failure. This can be, for example, with cirrhosis of the liver, when fluid accumulates in the abdominal cavity, which gradually spreads to other body structures, including the testes.

What is the problem

Dropsy testicles can be described as a sac surrounding a testicle filled with liquid, which is why the scrotum is enlarged in volume on one or both sides.In newborn boys, this problem is quite common and usually it disappears by itself in the first year of life. In the stronger sex, older than a year and a half, dropsy can form due to various factors, including injuries of the scrotum and groin, as well as inflammatory processes in the genitourinary system.

Symptoms of hydrocele: how is the disease manifested?

The symptomatology of the disease is determined by the type of dropsy, but there are also common symptomsamong which are the following:

  • the absence of pain during palpation of the testicles,
  • enlargement of the scrotum.

The congenital form of dropsy is characterized by changes in the size of the scrotum depending on the time of day: during the night, the fluid accumulated in the testicle membrane leaves, and during the day, on the contrary, accumulates in the scrotum. The appearance of such symptoms should serve as a reason for seeking medical attention.

It should be noted that hydrocele can be either unilateral or bilateral. In the first case, dropsy most often develops against the background of inflammatory processes in the testes and appendages. The second option is a manifestation of chronic pathologies of the genitourinary system or general edematous syndrome, developing as a result of liver or heart failure.

An acute hydrocele is suddenly developing, starting with a sharp soreness of the scrotum and a significant increase in the size of its affected half. In some cases, patients complain of signs of general intoxication (weakness, fever up to 38-39 degrees, lack of appetite, etc.).

In the absence of treatment, acute dropsy takes a chronic form, but often a chronic course is acquired initially. In this case, the patient complains of sensations of heaviness in the scrotum, as well as changes in its size during the day: an increase in the daytime and a decrease at night.

In the chronic form of the disease, the fluid between the testicle shells accumulates gradually, slowly, however, in the absence of medical care for a long time, the size of the scrotum may reach gigantic proportions (with a soccer ball), which significantly affects the quality of life of the patient, making it difficult to have sexual intercourse and urination, interfering with movement, and also creating problems when wearing underwear.

In addition, a significant increase in the volume of the scrotum leads to a violation of spermatogenesis and hypertrophy of its structures. The chronic process is characterized by a mild pain syndrome (dull aching pain) or its complete absence.

The lack of treatment or other adverse factors entail the development of complications of dropsy. The consequences may be as follows:

  • Testicular atrophy. The cause of the pathology is the compression of blood vessels that provide the testicle with blood, as a result of which its tissues are deficient in nutrients.
  • Accumulation of blood between the membranes. A complication develops as a result of unsuccessful dropsy puncture, with hemorrhagic diathesis, or as a result of mechanical damage to blood vessels.
  • Scrotal hernia.
  • Erectile disfunction.
  • Suppuration in the scrotum. At the same time, the general condition of the patient worsens, body temperature rises, the inflammatory process spreads to surrounding tissues.
  • Rupture of the testicles. This is due to their excessive stretching under fluid pressure.
  • Male infertility It develops as a result of a violation of spermatogenesis, which is caused by compression of the testicles.

Hydrocele treatments

Modern medicine involves treating dropsy of the testis in men in the following main ways:

That is why he is preferred in cases with patients aged, for whom full surgical treatment is strictly contraindicated.

The maximum positive effect and lasting result is achieved if dropsy of the testicles is treated by surgery. The official name of the operation is the plastic of the shells of the testicle, its implementation is usually carried out according to one of several common options:

  • Winkelman operation
  • Bergman’s operation

How to treat dropsy of the testicle with the choice of a specific type of intervention, the attending physician (surgeon) chooses in the process of performing the procedure. Types and causes of the disease

The main classification is the division into congenital and acquired species.

The disease appears either in the womb, or during adulthood. Sometimes the formation of clusters is not characterized by external causes and is manifested even in healthy adult men.

The main reason for congenital edema is when the testicles descend in the womb, the place that formed as a result of their movement should be overgrown. However, sometimes this does not happen. In the resulting pocket, dropsy develops. Usually this process occurs around the ninth month of pregnancy.

Preparation for surgery

To clarify the diagnosis, the doctor can prescribe the following studies:

Bergman's operation is carried out with a large testicle, with a chronic disease, thickening of the membranes. It is somewhat more traumatic than the Winkelmann modification, but it avoids the accumulation of tissues and the unaesthetic appearance of the testicle.

Operation Ross

This type of intervention is indicated for congenital hydrocele of the testis in children older than 2 years. The patient is lying on his back.

The doctor after the onset of anesthesia disinfects the inguinal zone and makes an incision in the lower abdomen. This is necessary in order to visualize the spermatic cord and not to touch it during further surgical intervention.

After that, the doctor discovers the vaginal process of the peritoneum, which is the cause of congenital hydrocele. He crosses education and bandages the stump. A hole (“window”) is formed in the inner shell of the testicle.

As noted in the Operational Urology manual (written by F. Hinman), “Lord's plication of the membranes is considered the most effective way to treat acquired testicular dropsy, with minimal trauma to the loose connective tissue of the scrotum.”

The patient is in a supine position. A doctor or nurse treats the lower abdomen and scrotum with an antiseptic. The surgeon stands on the right and pulls the affected testicle down to loosen the muscle that is lifting it.With the fingers of his left hand, he squeezes the spermatic cord and injects a solution of lidocoin into it. It serves to anesthetize.

The skin on the scrotum is stretched, and the doctor makes an incision about 4 cm long at the place farthest from the testicle. Affected blood vessels coagulate. Next, an incision is made to the inner (vaginal) membrane of the testicle. External tissues are pushed apart and fixed with clamps to prevent bleeding.

The wound has a rounded appearance, along its edges there are skin wrapped like a roller, subcutaneous tissue, and the outer (fleshy) membrane. After that, the surgeon makes a puncture of the vaginal membrane to evacuate the liquid contents. The vaginal membrane is also cut and squeezed into the wound testicle.

The doctor will examine it in detail to exclude concomitant pathologies. After this, the testicle rises and the membranes undergo plication (folding) - this is a feature of the Lord's operation. They are sutured with absorbable thread, which is then pulled. Then the testicle returns to its place, and additional necessary sutures are applied.

Consequences of the intervention

The following complications are possible depending on the type of operation:

  1. Hematoma (internal hemorrhage). As a rule, does not require treatment. A hematoma can occur when the sutures are loose.
  2. Relapse. Minimally invasive methods most often lead to repeated accumulation of serous fluid, if the cause of its formation is not eliminated. Some authors call the relapse rate up to 100%. In operations involving suturing of the testicle, such an accumulation is possible only if a pocket has remained around the parenchyma (loose tissue of the organ).
  3. Cosmetic defect. Acquired tissue with a strong hydrocele can have an unpleasant appearance of the so-called nodule. This can be avoided by resorting to excision surgery (Bergman). They are more traumatic, but nevertheless advisable when stitching a large area of ​​tissue is required.
  4. Swelling of the scrotum. It can be observed for several months and is not a cause for concern. The testicles should be soft to the touch.
  5. Excision of the spermatic cord. Such damage is very dangerous because it leads to infertility. The spermatic cord is practically not recoverable. Crucial is the speed of going to the doctor.
  6. Testicular atrophy. With this consequence of the operation, the organ gradually decreases in size, its ability to sperm is violated. Atrophy is not subject to treatment, testicular removal is indicated.

Recommendations for the recovery period are as follows:
